Exercise Participation Waiver

I acknowledge that participation in Fit-4-FurBabies events, including but not limited to boot camps, yoga classes, walks, runs, and fitness challenges, involves physical activity that may be strenuous and carries inherent risks of injury.

I voluntarily assume all risks associated with participation, including but not limited to falls, contact with equipment, and physical exertion. I certify that I am in good health and have no medical condition that would prevent safe participation.

I release Fit-4-FurBabies Foundation, its organizers, volunteers, sponsors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to my participation.

Medical Disclaimer

The fitness activities offered by Fit-4-FurBabies are not a substitute for professional medical advice, diagnosis, or treatment. Always consult with a qualified healthcare provider before beginning any new exercise program.

Fit-4-FurBabies recommends that participants obtain a physical examination from a physician prior to participation. If you experience chest pain, dizziness, or any other concerning symptoms during exercise, stop immediately and seek medical attention.

Photo & Video Release

By participating in Fit-4-FurBabies events, I consent to the photographing and recording of my likeness and voice. I grant Fit-4-FurBabies Foundation the irrevocable right to use my photo, video, and audio recordings for promotional, marketing, and educational purposes in any media.

I understand that I will not receive compensation for such use and that all materials become the property of Fit-4-FurBabies Foundation.

Donation Policy

All donations collected at Fit-4-FurBabies events are distributed to local animal shelters and rescue groups. Monetary donations are used for medical care, food, supplies, and operational needs of partner organizations.

Donations are non-refundable. Receipts for monetary donations are available upon request.

Communication Agreement

By registering for events or signing up for updates, I agree to receive email communications from Fit-4-FurBabies Foundation regarding events, volunteer opportunities, and community updates.

I may unsubscribe from communications at any time.
